Our Care Plans

At Animity Health, we provide care to help you feel better as soon as possible.

However, most musculoskeletal conditions require at least a short period of consistent care to improve, adapt, and stabilize. For that reason, our care is structured in phases to allow your body to respond appropriately—not just provide temporary relief.

Animity's approach is centered on functional restoration and symptom-guided care, followed by appropriate care to help sustain results over time and decrease frequency and severity of relapses.

In order to keep care straightforward, transparent, and focused on what is clinically appropriate, the office operates on a direct-pay model and does not bill insurance.

New Patients

New patient care begins with a focused initial phase designed to establish how your body responds to care over time. This phase is completed over the first few weeks and serves as the foundation for ongoing, consistent care.

Existing Patients:
Re-engagement

When there has been a gap in care, a short re-engagement phase is used to reassess your current condition and re-establish continuity. This allows care to resume appropriately rather than addressing symptoms in a single visit.

Single-visit re-engagement appointments are available for established pediatric patients (minors)

Routine or
Maintenance Care

After the initial phase, patients transition to care based on their needs.
For established patients, typical maintenance frequency: Every 4 to 6 weeks
To remain in routine/maintenance care: Visits must occur at least every 8 weeks

Wellness Subscription Plan

Wellness subscription plans are intended for established patients committed to consistent, ongoing care. This monthly recurring access retainer supports regular visit intervals and provides the greatest value for maintaining long-term care, regardless of how often visits occur within a given billing cycle. Monthly fees are fully earned upon charging to reserve ongoing clinical capacity and are non-refundable.

Maintenance Access Plans (MAP)

Maintenance Access Plans are designed for established patients committed to long-term health continuity. An upfront Concierge Access Retainer secures guaranteed practice capacity, priority scheduling privileges, and preferred member rates for your care over a dedicated 6-month or 12-month term. Retainers are fully earned upon receipt to reserve ongoing clinical capacity and are non-refundable.

The Fine Print:
All Maintenance Access Plan (MAP) retainers must be paid in full at initiation. Access plans and wellness subscriptions are individual and non-transferable; one plan per patient.

Concierge Access Retainers are non-refundable and fully earned upon receipt to cover allocated practice capacity, intake review, and operational availability. Unused plan visits expire at the conclusion of the dedicated access term (6 or 12 months) and do not roll over.

For Wellness Subscriptions, monthly auto-pay fees are fully earned upon charging to reserve ongoing clinical capacity and are non-refundable. Unused monthly subscription visits do not roll over. Cancellation of monthly auto-pay requires 30 days’ advance written notice, submitted either via email to info@animityhealth.com or in person at our office using our auto-pay cancellation form.

Plan holders must meet minimum visit frequencies as outlined in their selected plan. Additional visits beyond plan minimums may be used at the active plan’s equivalent rate within the plan term. Plan renewal rates are subject to change upon expiration of the initial term.

Preferred rates are available for established patients age 65 and older. Inquire at the office for senior access plan details.
