Our Care Plans

At Animity Health, we provide care to help you feel better as soon as possible.

However, most musculoskeletal conditions require at least a short period of consistent care to improve, adapt, and stabilize. For that reason, our care is structured in phases to allow your body to respond appropriately—not just provide temporary relief.

Animity's approach is centered on functional restoration and symptom-guided care, followed by appropriate care to help sustain results over time and decrease frequency and severity of relapses.

In order to keep care straightforward, transparent, and focused on what is clinically appropriate, the office operates on a direct-pay model and does not bill insurance.

New Patients

New patient care begins with a focused initial phase designed to establish how your body responds to care over time. This phase is completed over the first few weeks and serves as the foundation for ongoing, consistent care.

This initial phase establishes how your body responds to care over time and serves as the foundation for ongoing, consistent care.

Existing Patients:
Re-engagement

When there has been a gap in care, a short re-engagement phase is used to reassess your current condition and re-establish continuity. This allows care to resume appropriately rather than addressing symptoms in a single visit.

Returning Patients (After 2+ Months)

This ensures care is restarted and managed appropriately over several treatments.

Single-visit re-engagement appointments are available to established child patients.

Routine or
Maintenance Care

After the initial phase, patients transition to care based on their needs.
For established patients, typical maintenance frequency: Every four to six weeks
To remain in routine/maintenance care: Visits must occur at least every eight weeks

The fine print: Membership must be paid in full.  Memberships may not be shared.  One membership per patient.  No refunds for unused portions of club packages.  Unused monthly visits do not roll over.  Members must use visits per plan requirement.  Members may use visits more frequently than the plan requirement and may renew their membership at the effective club rate at the time of renewal.  Membership rates subject to change at any time.  Monthly auto-pay non-refundable after payment has been charged.  Cancellation of monthly auto-pay requires 30 day advanced notification in writing by either email to info@animityhealth.com or in person at our office by completing the auto-pay cancellation form.
